CC25I Pre-Implementation County Self-Assessment and Planning Tool - This is a comprehensive tool for data and systems evaluation of a community's transition-aged youth-serving system across the 7 key CC25I focus areas. This self-assessment guides development of locally designed plans for implementing CC25I.
CC25I Logic Model - A comprehensive one-page view of the Initiative in matrix form including CC25I focus areas, funding inputs, goals, typical activities, and anticipated outcomes.
CC25I Outcomes Framework - A chart of the CC25I outcomes, performance indicators and age ranges at which applicable. Note that the performance indicators in bold are "priority" indicators which all CC25I counties are tracking at start-up of the initiative.
CC25I At Emancipation Assessment - This assessment has been built into our ETO database in order to track outcomes for youth at the point of emancipation from foster care.
