California Connected by 25: A California Family to Family Strategy
Building Partnerships & Improved Outcomes for Transitioning Foster Youth

California Connected by 25 Initiative (CC25I) is a collaborative effort of five foundations assisting public child welfare agencies and their communities to build comprehensive transition-aged foster youth supports and services for youth 14 through 24. Through comprehensive assessment, planning and practice innovations developed with youth, caregivers, and other partners, effective strategies are being developed that can be spread to improve transition practice and outcomes statewide. An outcomes framework and Efforts to Outcomes (ETO) database align with the 7 key focus areas of our Logic Model to provide critical information for on-going self-evaluation and program improvement. The data collection strategy also assists with ILP services and incentives tracking and provides date for the quarterly county "At Emancipation" data report. Youth engagement, technical assistance and twice-yearly convenings are important in creating the learning environment that supports successful implementation of Connected by 25 in California.

CC25I Introductory Report - This is a comprehensive overview of the Initiative and the context within which it is moving forward. The report includes profiles of the five California counties who began implementation during 2005/2006. For a condensed version of this comprehensive report, a Summary Report is provided.

CC25I Pre-Implementation County Self-Assessment and Planning Tool - This is a comprehensive tool for data and systems evaluation of a community's transition-aged youth-serving system across the 7 key CC25I focus areas. This self-assessment guides development of locally designed plans for implementing CC25I.

CC25I Logic Model - A comprehensive one-page view of the Initiative in matrix form including CC25I focus areas, funding inputs, goals, typical activities, and anticipated outcomes.

CC25I Outcomes Framework - A chart of the CC25I outcomes, performance indicators and age ranges at which applicable. Note that the performance indicators in bold are "priority" indicators which all CC25I counties are tracking at start-up of the initiative.

CC25I At Emancipation Assessment - This assessment has been built into our ETO database in order to track outcomes for youth at the point of emancipation from foster care.

Financing Workforce Development Programs for Youth Transitioning Out of Foster Care - This Connected by 25 brief explores six financing strategies that can support workforce development programs and services for youth currently in or transitioning from foster care. The brief describes strategies for maximizing workforce development and child welfare resources; accessing community development resources; connecting with education resources; building partnerships with employers; and improving coordination across systems.

Financing Entrepreneurship Programs for Youth Transitioning Out of Foster Care - This Connected by 25 strategy brief aims to help policymakers, child welfare administrators, program developers, and community leaders understand the funding sources and financing strategies that can support entrepreneurship opportunities for youth currently in or transitioning from foster care.

Financing Asset-Building and Financial Education Programs for Youth Transitioning Out of Foster Care - This Connected by 25 brief describes five financing strategies that policymakers, program developers, and community leaders can pursue to support asset-building and financial education programs for youth in foster care. For each strategy, the brief includes funding sources, examples of how asset-building and financial education programs have used these resources, and considerations for implementation.
